Purpose:To compare the efficacy of fluconazole (Fluconazole) and diflucan (Diflucan), two oral antibiotics used to treat cryptococcal meningitis (CMA) with fluconazole (Fluconazole) and diflucan, and with fluconazole and diflucan. Methods: A retrospective cohort of patients with CMA, who received fluconazole and diflucan for two years, and who had not received fluconazole or diflucan for CMA at any time, were compared for efficacy using the Cochrane Collaboration’s Randomized Controlled Trial (RCT), the International Prognostic Registry for CMA (IPP) study. Results: In the RCT, overall survival rates were significantly increased for fluconazole (RR 0.41; 95% CI 0.19–0.69), diflucan (RR 0.42; 95% CI 0.20–0.89) and fluconazole (RR 0.42; 95% CI 0.19–0.76) but not for diflucan. The incidence of death was increased with fluconazole (RR 1.14; 95% CI 0.90–1.49) and diflucan (RR 0.57; 95% CI 0.32–1.10) and not with fluconazole (RR 0.22; 95% CI 0.02–0.99) but not with diflucan. The incidence of major adverse events, including headache, nausea, vomiting, abdominal pain, and anorexia, was increased with fluconazole and diflucan and not with fluconazole. The mean survival was not significantly different between the two groups and was comparable for both groups. In this retrospective cohort, the overall mortality rate was significantly higher for fluconazole than diflucan (RR 0.59; 95% CI 0.31–0.89) and the incidence of major adverse events was significantly higher for fluconazole than diflucan (RR 1.03; 95% CI 0.94–1.22) and fluconazole (RR 1.11; 95% CI 0.86–1.38). The mean number of opportunistic infections was significantly higher for fluconazole than diflucan and fluconazole (RR 0.87; 95% CI 0.62–1.27) but not for diflucan (RR 1.02; 95% CI 0.74–1.37). The incidence of opportunistic infections was significantly higher for fluconazole than diflucan and fluconazole and not for diflucan. The mean time from first exposure to opportunistic infections was not significantly different between the two groups and was comparable for both groups. Results: This study showed no statistically significant difference in overall mortality between fluconazole and diflucan. The absolute risk difference was statistically significant (RR 1.02; 95% CI 0.85–1.22). The absolute risk difference was clinically insignificant. The incidence of major adverse events was increased with fluconazole (RR 1.03; 95% CI 0.88–1.13) and diflucan (RR 0.85; 95% CI 0.65–1.25) and not with fluconazole (RR 0.19; 95% CI 0.02–0.98). There were no clinically significant differences in the absolute risk difference between fluconazole and diflucan. The mean survival was not significantly different between the two groups. The incidence of major adverse events was increased with fluconazole (RR 1.14; 95% CI 0.90–1.46) and diflucan (RR 1.08; 95% CI 0.82–1.33) and not with fluconazole (RR 0.77; 95% CI 0.45–1.12). The incidence of opportunistic infections was increased with diflucan (RR 1.14; 95% CI 0.90–1.47) and not with fluconazole (RR 0.85; 95% CI 0.65–1.27). The mean time from first exposure to opportunistic infections was not significantly different between the two groups. The absolute risk difference was statistically significant (RR 1.
